DESCRIPTION:The strong interaction of quarks and gluons is at the origin o
 f almost all of the mass of the visible universe. The underlying quantum f
 ield theory\, Quantum Chromodynamics (QCD)\, is a consistent part of the h
 ighly successful Standard Model of elementary particles. The emergence of 
 phases and structures in QCD\, from quarks and gluons to mesons and baryon
 s\, to atomic nuclei and neutron stars\, is one of the persistently fundam
 ental issues in modern science. This colloquium reviews our current unders
 tanding of the phases of QCD. Empirical information from nuclear collision
 s at the highest available energies will be surveyed together with results
  of Lattice QCD thermodynamics. Important constraints on the phase diagram
  come from nuclear physics and the treatment of the nuclear many-body prob
 lem using modern effective field theory approaches based on the symmetry b
 reaking pattern of low-energy QCD. The presentation concludes with a discu
 ssion of new conditions for the equation-of-state of dense baryonic matter
  derived from the recent observation of massive (two-solar-mass) neutron s
